Referrals
- Referral forms are available for the Social Worker to complete via Liquid Logic. The Emotional Health and Wellbeing Referral Form can be accessed in the ‘Forms’ tab on the service user’s record.
- All referrals must be supported by a Strength and Difficulties Questionnaire (SDQ) completed within the last two months by either the child or young person themselves or a professional working with them.
- A referral will not be accepted by Polaris Children’s Services without the completed SDQ attached.
Consultation
All referrals made to the service, will receive a consultation to inform decision making and the pathway of intervention appropriate for the child/young person and their parents/carers.
The outcome of the consultation is uploaded to the case management system on Liquid Logic by Polaris Children’s Services’ staff.
